Please help us to be well prepared for your mission trip! We will begin to gather your Group Information with our first conversation and fill in as the details of your week develop. We ask that Volunteer Registrations and Minor Consent forms be submitted at least 30 days prior to your adventure.

PARENTAL CONSENT FORMS FOR MINORS
For ages 14 through 18. Must be completed by Parent or Court Appointed Legal Guardian and must be submitted, along with the Volunteer Registration form, prior to the participant's arrival on project.
